Українці, які мають протипоказання до вакцинації, можуть отримати відповідну довідку. Форму довідки раніше затвердило МОЗ.

Про це повідомляє Центр громадського здоров’я у Facebook.

Форму № 028–1/о заповнюють, ґрунтуючись на медичних записах про здоров’я пацієнта.

Отримати довідку можуть люди, які мають медичні протипоказання до щеплень. Раніше було складено перелік постійних та тимчасових протипоказань. Ознайомитись з ним можна нижче.

МОЗ склало інструкцію щодо заповнення довідки.

У довідці вказують ПІБ пацієнта, його дату народження, інформацію про лікаря, який видав довідку, дані про постійні або тимчасові протипоказання, термін дії довідки. Лікар повинен поставити підпис та печатку медичного закладу.
